Important Practice Update

After careful consideration, Kerry Palakanis, DNP, APRN, FNP-C has made the decision to transition her practice and retire from providing primary care services effective October 31, 2026.

As part of this transition, she will also be discontinuing participation with health insurance plans. Patients are encouraged to contact our office regarding how this change may affect appointments or services scheduled prior to October 31, 2026.

It has been her privilege to serve as a primary care provider for over 35 years. She is sincerely grateful for the trust you have placed in her and for the opportunity to be part of your healthcare journey.

To help ensure continuity of care, current patients should begin establishing care with a new primary care provider as soon as possible. Our office will assist with the transition of your care and the transfer of medical records upon receipt of an appropriate authorization.Please do not wait until your final prescription refill or an urgent medical need arises to establish with a new provider. Allowing adequate time for this transition will help prevent interruptions in medications, follow-up care, referrals, and other healthcare needs.

My final day providing primary care services will be October 31, 2026.Thank you for allowing me the privilege of caring for you. I am deeply appreciative of the relationships I have developed with my patients over the years and wish each of you continued health and well-being. 

Although I am retiring from primary care, I am not retiring from healthcare practice entirely.

I will continue to offer a limited selection of specialized and elective healthcare services on a cash-pay basis. I will no longer bill health insurance for these services.

Services that I anticipate continuing to provide include:

• BioTE® Hormone Replacement Therapy
• Testosterone Replacement Therapy (TRT)
• Medical Weight-Loss Services
• Laser and Cosmetic/Aesthetic Treatments
• Other Selected Wellness and Limited Healthcare Services

Patients may elect to continue seeing me for these services; however, participation in these services does not replace the need to establish and maintain a relationship with a primary care provider.

My continued treatment of a patient for hormone therapy, testosterone therapy, weight management, cosmetic treatments, or another limited service should not be interpreted as my continuing to serve as that patient's primary care provider.
